Mission

Inspired by our city’s history, we will produce new physician leaders blending science and emerging technology with compassion, collaboration and advocacy to guide patients, families and our region to better health and future well-being.

Program Aims

  1. Residents will have rotation experiences in each year and longitudinally through continuity of care for a panel of patients that will introduce and promote the use of remote monitoring, virtual consultation and virtual home and hospital visits.
  2. Residents will have both inpatient and urgent experiences with acute and chronic behavioral health problems in patients. Emphasis in clinic with faculty will focus on triggering of patients and treatment and counseling using both team and individual approaches to promote resiliency of patients.
  3. Residents will have exposure longitudinally using EMR data to improve care and to perform Quality Improvement in the practice. PGY-3 residents will have a specific health management and research rotation working with QI experts within the health system to identify a topic of interest, develop a data collection strategy with literature review and design an intervention that will be monitored and reported to the faculty and interested management. Longitudinally the QI team will work with the clinic data to review individual and group performance.
  4. Throughout the 3 years and in specific rotational experiences the residents will have active teaching on transitions of care and handoffs emphasizing the use of technology and EMR to ensure accurate and timely follow-up and medication reconciliation. When possible, a team approach utilizing nursing and social services staff will teach the handoff and transition management pitfalls.
  5. Using faculty with expertise in chronic disease management protocols (emphasis on diabetes and CV disease), residents will be taught the approach to chronic disease management maximizing the use of teams of care and patient engagement. Home monitoring and virtual communication methods will be emphasized.
  6. Through a series of readings, lectures, data mining and community experiences throughout the program and specifically in the Community Medicine rotation, residents will understand the social determinants related to poor health outcomes for their patients and the community at large, and with the QI process and community resources will explore practical advocacy and social interventions that a practicing physician could employ in their community.
