Claiborne Health and Rehabilitation Center is the only five-star nursing home in Claiborne County. Our full range of services includes both short-term care and rehabilitation after surgeries or hospital stays, and long-term care for loved ones who can no longer live independently.

In November 2024, Claiborne Health and Rehabilitation Center received two national recognitions as a “Best Nursing Home”. Medical Care and Insurance

Medical care is provided under the direction of primary care physicians. We are located adjacent to Claiborne Medical Center, so residents have immediate access to emergency and hospital services if needed.

Claiborne Health and Rehabilitation Center accepts most commercial, Medicare, and TennCare plans. We also accept private pay. We are not in network with the Veterans Administration (VA).

Health and Rehabilitation Center Visitation Guidelines:

Our health and rehabilitation center currently allows visitation by families and friends of residents. We kindly ask that all visitors follow these guidelines in order to prevent the potential spread of COVID-19 and other infectious diseases:

  • Perform frequent hand hygiene, including when entering and exiting the facility.
  • Contact our facility directly at 423-526-2188 regarding current mask requirements and other visitation guidelines.

Please note, these guidelines may be amended based on a) a patient-care decision by our administration, b) during a declared public health and civil preparedness emergency, or c) in case of a natural disaster. During exceptions, we will follow state or federal regulatory guidelines for visitation. These guidelines will be available for reference.

Protect Yourself and Help Us Protect Our Residents and Staff! If you have symptoms such as fever, cough, a runny nose or sore throat, please consider postponing your visit. If you have any of the symptoms listed or similar ones associated with illnesses like cold and flu, please wear a mask and remain in the room with the resident. Limit travel within the nursing home to the first-floor visitation and activity area.
