Reporting a Lost Item or Guests Checking for an item they’ve lost

If someone (patient, guest, or employee) needs to report something lost to Lost & Found do so by:

• Filling out the online form (QR code sheets are available at all entrances, or the link is here: https://www.covenanthealth.com/parkwest/lost-found/ ) or form available below.
• Call the lost & found number and leave a message: 865-373-2905

Unfortunately, our PCU waiting team cannot assist people with lost and found matters when they present at the PCU waiting desk. Please do not instruct guests to go to PCU waiting for any lost and found matters. The team in PCU is not the lost and found team.

What to do with found items:

If you find items in your department, please follow the departmental/facility process:

• If you know whose it is (past patient, etc.) please try to contact them to let them know you have the item. You can arrange for them to pick it up from you in your department for the first 48 hours of possession.
• If they do not come pick it up in the first 48 hours, please label with all information you have (patient name, room number, date found, attempts to contact, any notes of conversation with the patient/family) and place the item in your departmental lost & found pick-up box. The lost and found team will pick these items up at least once a week.
• If you feel the item is over $50 in value, please make the initial contact with the patient if you know who it belongs to and contact the House Supervisor to place the item in the safe. We’ll still need all the info and details your team might have about the item, including conversations with the family about picking it up. If you leave a message for the family, you can let them know that they’ll have to ask for a House Supervisor to pick up item from the safe when they arrive.
