About FamilyCare Specialists

At FamilyCare Specialists, we specialize in the treatment of the whole patient across all stages of life – from age-appropriate immunizations, wellness exams, and acute illnesses like colds and flu, to caring for your chronic health conditions like diabetes and high blood pressure. With more than a century of combined medical experience, our expert healthcare team is ready to provide you with excellent care. Whether you’re sick with a common cold or dealing with with a life-long condition, we’re prepared to help you feel better. We offer primary medical care close to home, so you get the right care at the right time and place.

Walk-In Clinic

Please visit our walk-in clinic for these conditions, which require same-day or immediate care but are NOT life-threatening. Our walk-in clinic is open Monday – Friday, 7 a.m. – 5:00 p.m.

CMG Connected Care Management

We believe healthcare is about more than a medical office visit for an illness or immediate need. FamilyCare Specialists is pleased to offer CMG Connected Care Management to help you manage chronic conditions and optimize your health. CCM services are in addition to the medical care you receive at our office. Your CCM Care Team partner will help manage your care plan, make sure you receive timely care, and connect you with additional services when needed. Learn more about Connected Care Management.

When to Seek Emergency Medical Care

Please note we cannot treat these conditions at our primary care office or walk-in clinic. Call 911 or go to the nearest emergency room for treatment.

  • Bleeding that will not stop
  • Heart/cardiac issues – heart attack/pain
  • Extensive or complicated cut/laceration
  • Extreme abdominal pain
  • Fainting or head injury
  • Injury or illness of infant under 1 month of age
  • Laceration requiring stitches in young children
  • Loss of consciousness or disorientation
  • Loss or sudden change in vision
  • Major fracture
  • Obstructive breathing or extreme shortness of breath
  • Rabies exposure/animal bite
  • Seizure
  • Serious burn
  • Severe rectal bleeding
  • Spinal injury
  • Stroke, numbness and/or facial drooping
  • Sudden change in mental status

Patient Forms

Prepare for your visit by filling out these forms ahead of your appointment and bring them with you. This will help us provide you with an efficient visit. Thank you!

Visit Reminders

We ask that you please arrive at least 30 minutes prior to your scheduled appointment. Please bring the following items with you to your visit:

  • Your current health insurance information

  • Your current Photo ID

  • Any medications you are currently taking in their original containers or in a list with dosages and times

Please note: In order to provide efficient care for all of our patients, you may be asked to reschedule your appointment if you do not arrive at least 30 minutes early.

Prescription Refills

It’s very important that you request your prescription refills during normal business hours before 4 p.m. Monday-Friday. Prescriptions will not be refilled on weekends or holidays.

Online Scheduling

At FamilyCare Specialists, online appointments are available for primary care visits.

We also offer the option to hold your place in line at our walk-in clinic. We’ll reserve your place in line so that you can wait comfortably at home until your estimated visit time.

Telehealth

Telehealth — sometimes called telemedicine — lets patients receive medical care virtually without an in-person office visit. Telehealth is done primarily online with internet access on your computer, tablet, or smartphone.

You can schedule a virtual appointment by calling our office or by requesting a visit through our secure patient portal, MyCovenantHealth. Your provider will review your current condition to determine if a telehealth appointment is right for you.
