Covenant HomeCare’s rehabilitation team helps patients become as functionally independent as possible. Rehabilitation may be prescribed as part of recovery from injuries, stroke, wounds, or other conditions. Our team uses a multi-faceted approach to ensure the health of our patients and achieve rehabilitation goals in a timely manner. 

What Are Rehabilitation Services?

Rehabilitation services focus on on improving the skills and functions necessary for daily living that have been lost because of illness, injury, or disability. Although patients may not be able to return fully to their previous condition, rehabilitation helps them regain as many abilities as possible. 

The Rehabilitation Team

Rehabilitation teams are structured according to a patient’s specific needs. Team members may vary, but usually include these medical and health professionals:

Our professionally certified team members are committed to the highest quality of care. We also have therapists with specialized certifications for lymphedema therapy, vestibular (balance) rehabilitation, and VitalStim therapy for patients who have difficulty swallowing.

In addition to medical and health professionals, a patient’s partner and/or family members also play a critical role in effective treatment outcomes.

Specialized Rehabilitation Care

Stroke Rehabilitation

Covenant HomeCare’s stroke rehabilitation program is a specialized program to help patients recover at home from the effects of a stroke. Although recovering from a stroke can be challenging, the rehabilitation team assists the patient in becoming as functionally independent as possible.

Patients appropriate for our stroke rehabilitation program include those who have a primary diagnosis of stroke or have a stroke-related disorder such as paralysis, weakness, balance disorders, and dysphagia (difficulty swallowing). Our rehabilitation treatment is based on national standards and evidence-based practices that include careful planning, goal setting, and effective, intensive one-on-one therapy.

Our licensed, experienced staff evaluates and treats patients promptly after receiving a physician’s referral, and we communicate regularly with your physician about your progress. We also evaluate and help obtain any prosthetic/orthotic, assistive device, or adaptive equipment that might be needed.

Wound Management

Covenant HomeCare offers a comprehensive wound management program. We have certified wound care consultants on staff who have completed intensive training to assure high-quality care. Our wound care clinicians work with your physician to develop a plan of care that uses the most up-to-date nursing interventions and wound care products for optimal healing to occur. 

Our wound management program includes:

  • Detailed assessment and care of acute and chronic diabetic, venous, arterial, pressure, and surgical wounds.
  • Wound healing progression tracked through digital photography, with copies sent to the physician.
  • Vascular Dopplers available to perform Ankle Brachial Index (ABI) for comprehensive compression therapy.
  • Wound care clinicians trained in Wound VAC (negative pressure) therapy to encourage healing
  • Extensive education provided for patients and families in wound prevention, appropriate skin care, pressure relief surfaces, nutrition to promote healing, and good skin care.
