How We Care
Hospice at Covenant Health is a team effort. Nurses, physicians, social workers, chaplains, counselors, and trained volunteers work together around your loved one’s goals, not a standard protocol.

Your Care Team
Every patient is supported by a full team. Here is who will be there for your family.
The Four Levels of Hospice Care
Every patient is different. These four care levels allow us to adjust support as needs change.
Routine Care
The most common level of hospice care. Our team visits your home on a regular schedule to manage symptoms, provide nursing care, and check in on your family. The frequency of visits is based on your loved one’s needs.
Continuous Care
When a patient is going through a difficult period and needs more support, our team can provide extended care at home, including overnight visits, to help stabilize symptoms and keep your loved one comfortable.
Respite Care
Caring for a loved one is demanding. Respite care gives family caregivers a short period of rest by arranging care for your loved one at a partnering facility for up to five days. This is not a step back in care. It is a way to help your family sustain the caregiving role.
General Inpatient Care
If symptoms cannot be managed at home, your loved one may receive a short stay at a partnering facility where the care team can address more complex needs. Once symptoms are under control, care returns home.
