Important Appointment Reminders:
- Please arrive at the clinic 15 minutes before your scheduled appointment time unless you are asked to arrive earlier.
- Children under the age of 18 years are not permitted in the Covenant Health Oncology Group Offices.
- Please bring a photo ID, your insurance cards, and your designated co-pay to each visit.
- Please make sure our office has your updated phone numbers and email. *Let us know if you prefer appointment reminders via text message.
- In an effort to prevent the spread of illness, we follow Covenant Health COVID-related visitor restrictions.*No visitor with any illness (fever, cough, shortness of breath, etc.) should come to the clinic, and any patients with these symptoms should call before coming in for direction. Please help us protect our immunocompromised patients by following these guidelines.

If you have scheduled your appointment with Covenant Health Oncology Group, welcome to our practice, and thank you for trusting us to be part of your health care team. Sharing accurate information with your care providers is very important, but trying to complete forms and recall your health history and medications as you sit in the lobby before your appointment can be overwhelming.
Save time at your appointment and complete your new patient paperwork in advance.
Please take advantage of the opportunity to complete your paperwork in advance and bring it with you. This option allows you to take your time and make sure the information you provide is accurate and complete. When it is time for your appointment, you can focus on your meeting with your physician and any questions you may want to ask.

- Call 865-331-2060 to request that your paperwork be sent to your email. You can complete and submit forms online through a secure, HIPAA-compliant system prior to your appointment.
- Please download your forms for your New Patient Paperwork. You can print and complete them at your convenience. Do not forget to bring them with you to your first appointment.
