Requesting Your Medical Records 

For all medical records requests, a copy for your state-issued photo ID is required.  

Covenant Health is committed to protecting your health information and ensuring it’s available when you need it. Our Centralized Release of Information Department is here to make the process simple, secure, and convenient.   

Our Centralized Release of Information Department processes all medical records requests for our 10 hospitals* and many of our clinics. View the full list of facilities we process requests for.

    Option 1: Printed Request 

    1. Download and print the authorization form. 
    2. Fill out the information. 
    3. Send your completed form to Covenant Health.

    Option 2: Digital Request 

    If you prefer to sign and submit your request electronically, follow these steps: 

    1. Call 865-374-5269 to request a secure form. We will email you the form. 
    2. After receiving your form, fill out all information. 
    3. Submit your form by clicking the “complete” button on the form itself.

    Option 3: MyCovenantHealth Portal 

    1. Login or enroll in the MyCovenantHealth online portal. 
    2. On the home page, select “Health Record” on the top menu bar. 
    3. A new menu will open with different record options. Select the record you would like to access. 
    4. Click “Download” on the document page that opens. 
    5. Check the downloads on your device to view your record.

We understand this can be a difficult time, and we want to make accessing records as simple as possible. To help guide you through the process, we have developed a records request packet that includes:

Note: We strive to process requests within 10 days. Applicable fees may apply per federal and state guidelines. 

Requesting Medical Records for an Upcoming Appointment 

If you have an upcoming appointment and need copies of your medical records or imaging: 

Birth Certificate Requests 

Our team does not process birth certificate requests. These requests must go through the Tennessee Department of Health. You can view their certificate request instructions for more information.  

*For imaging records from Claiborne Medical Center only, please call 423-526-2331. For all other Claiborne Medical Center records, follow the steps above.
