For people living with ongoing health conditions, having support between visits can make all the difference. That is why Covenant Health started the Connected Care Management program to provide ongoing care, guidance and connection.  

This program is designed to help patients stay supported, address concerns early and feel confident managing their health. 

What is Connected Care Management? 

Connected Care Management is a service offered through Covenant Medical Group for patients with chronic health conditions. The program focuses on coordination, education and regular communication to help patients stay informed and supported. 

Patients enrolled in the program work with a care manager to create a personalized plan. Each plan supports medication management, symptom monitoring, appointment coordination and communication with their primary care provider. 

Care is delivered through phone calls and follow-up visits that fit into everyday life. Patients receive consistent support without additional travel or extra appointments. 

How the Program Makes Life Easier 

  • Hospital diversion: Frequent check-ins and close teamwork with your provider help catch concerns early. This can prevent emergency room visits and reduce hospital readmissions. 
  • Health and safety: Staying home helps lower the risk of infections and complications that can happen during hospital stays. Patients receive guidance before small issues grow into bigger problems. 
  • Quality of life: Remaining at home allows patients to stay independent and active in their daily routines. 
  • Caregiver support: Care managers help organize care, reduce duplicate appointments and improve communication. This takes pressure off loved ones. 

Meet the Heart Behind the Care: Randi Beaty 

Randi Beaty, LPN, has been with Covenant Health for six years and part of the Connected Care Management program for two years. She works closely with patients to build trust, listen carefully and provide steady guidance. 

“This role in Connected Care Management allows me to connect with my patients on a deeper level,” Randi explains. “Without the constraints of a fixed appointment time, I can provide my full, undivided attention and deliver more personalized care.”
