At Covenant Health, our cardiac rehabilitation program is designed to help you regain strength, improve heart function, and enhance your overall quality of life after a cardiac event or procedure. Depending on your unique needs, the program may last from six weeks to a year or longer, with a personalized approach to exercise, education, and lifestyle changes. Our goal is to support your recovery, reduce your risk of future heart issues, and help you confidently return to your daily activities.

Phase 1: Inpatient Rehabilitation

While recovering in the hospital, you’ll begin with light, supervised activities paired with personalized education on heart health, medication management, and lifestyle changes to promote recovery and build confidence for your return home.

Phase 2: Outpatient Assessment

After discharge, our team designs a personalized exercise and education program tailored to your specific needs, with monitored workouts, heart rate tracking, and ongoing guidance from our cardiac specialists to ensure safe progress and improved heart health.

Phase 3: Outpatient Education and Exercise

Patients continue supervised exercise sessions, receive personalized guidance on lifestyle changes, nutrition, and stress management, and are equipped with ongoing support to help maintain long-term heart health and prevent future complications.

Phase 4: Independent Monitoring

Graduates of the program maintain their exercise routines independently while receiving periodic check-ins from our team to assess their progress, provide support, and ensure they stay on track with their long-term heart health goals.

Who Is Eligible for Cardiac Rehabilitation?

Cardiac rehabilitation is recommended for individuals who have experienced certain heart conditions or procedures, including:

  1. Heart Attack (Myocardial Infarction) – Within the past 12 months
  2. Coronary Artery Bypass Graft (CABG) Surgery
  3. Stable Angina (chronic chest pain due to coronary artery disease)
  4. Heart Valve Repair or Replacement
  5. Percutaneous Coronary Intervention (PCI) or Stenting
  6. Heart or Heart-Lung Transplant
  7. Chronic Heart Failure with Reduced Ejection Fraction (HFrEF ≤35%)
