In support of this philosophy, if you are an uninsured person with no public or private source of payment for medical services, Covenant Health will provide, at a reduced rate, medically indicated services.

To apply for financial assistance, please click the Financial Assistance Application and Instructions link located below (available in English and Spanish), print it, and completely fill out the information. The form also asks for supporting documentation as follows:

You May Be Able to Get Supplemental Security Income (SSI)

What is SSI?

Supplemental Security Income (SSI) is a federal program that provides monthly payments to people who have limited income and few resources. SSI is for people who are 65 or older, as well as for those of any age, including children, who are blind or have disabilities. To get SSI, you must meet one of these requirements:

How Much Can You Get?

The basic monthly SSI payment for 2025:

Note that not everyone gets the same amount. You may get more if you live in a state that adds money to the federal SSI payment. You may receive less if you or your family has other income. Where and with whom you live also make a difference in the amount of your SSI payment. Amounts are subject to change annually.

How Do You Apply for SSI?

If you plan to apply for SSI, you can begin the application process at SSA.gov/SSI. You can also call toll-free at 1-800-772-1213 to set up an appointment with your local Social Security office. If you are deaf or hard of hearing, you can call the TTY number: 1-800-325-0778.
