About Cumberland Diabetes Center

At Cumberland Diabetes Center, our goals are to give you access to the care you need and to improve your overall health. And we do so through medical, nursing, and dietary services delivered by our dedicated team of physicians, nurses and health professionals.

Cumberland Diabetes Center was established by John Sherrill, MD, who also serves as our current director. Dr. Sherrill has a passion to leave a legacy of better health for the community. Cumberland Diabetes Center is recognized by the American Diabetes Association.

Diabetes and Obesity Management Services

Cumberland Diabetes Center was established in 2019 for the purpose of bettering the quality of life of patients affected by diabetes or obesity.

Our program is available to all patients and their families who are motivated to develop knowledge and self-care skills. We empower patients with individualized education, goals, and self-management mentoring.

  • Education covers nutrition, exercise physiology, evidence-based pharmaceutical therapy, behavioral health, weight loss, and tobacco cessation.
  • Self-management plans are built around a patient’s lifestyle, co-morbid conditions, bio-markers, motivations, mental health, and socioeconomic circumstance.

Please note a physician referral is required for diabetes management but not for weight loss. We offer:

  • Pre-diabetes management
  • Education and management of newly diagnosed diabetes
  • Intensive care of Type 1 or 2 diabetes
  • Weight loss management

Our Mission

Our mission is to provide a team-based care center that leverages all available resources and innovations to motivate, educate, train, and treat diabetes and obesity. We’ll collaborate with providers, patients, and the community to provide the best possible care for the prevention, management, and potential remission of diabetes. We’ll empower patients to achieve and sustain their best quality of life.

Our Goal

Our goal is to support referring providers by coming alongside their patients with focused education and/or intensive management. We want to help prevent progression or complications from diabetes and obesity. To achieve sustainable lifestyle changes and disease management, patients need continuous monitoring, guidance, and support.

Caring for Our Community

Our patients deserve excellent medical care. It’s our commitment at Cumberland Diabetes Center to provide the best possible care to educate and support individuals affected by diabetes or obesity. To truly manage patients holistically, the education and self-care skills can’t be contained to the four walls of the office, but must be include family, the workplace, school, place of worship, and the community.

Visit Reminders

We ask that you please arrive at least 30 minutes prior to your scheduled appointment. Please bring the following items with you to your visit:

  • Your current health insurance information

  • Your current Photo ID

  • Your completed registration forms

Please note: In order to provide efficient care for all of our patients, you may be asked to reschedule your appointment if you do not arrive at least 30 minutes early.

We want you to be an active participant in your healthcare and encourage you to:

  • Provide information about your healthcare needs, preferences, values, and goals
  • Share your complete medical history and any changes to your health
  • Ask questions
  • Provide feedback about your treatment plan, including any problems with following the plan or reaching your health goals
  • Contact us with any health concerns
Prescription Refills

It’s very important that you request your prescription refills during normal business hours before 4:30 p.m. Monday-Thursday. Prescriptions will not be refilled on weekends or holidays.

You can request a refill on the MyCovenantHealth Patient Portal or by calling your pharmacy.

Telehealth

Telehealth — sometimes called telemedicine — lets patients receive medical care virtually without an in-person office visit. Telehealth is done primarily online with internet access on your computer, tablet, or smartphone.

You can schedule a virtual appointment by calling our office or by requesting a visit through our secure patient portal, MyCovenantHealth. Your provider will review your current condition to determine if a telehealth appointment is right for you.
