Claiborne Health and Rehabilitation Center Welcomes UHealth Providers

Claiborne Health and Rehabilitation Center is proud to welcome the UHealth team as our new on-site medical providers. UHealth specializes in delivering high-quality medical care to nursing home residents, and we are excited to introduce their dedicated team at our facility.

Dr. Paul Swafford will serve as the Medical Director of Claiborne Health and Rehabilitation Center, bringing his expertise to our residents 1–2 days per week. Supporting him are nurse practitioners Malinda Munsey and Pamela Scholl, who will be on-site five days a week to provide consistent, hands-on care. This level of provider presence is significantly higher than what is typically available in nursing homes and ensures our residents receive the timely care they need and deserve.

In many facilities, the absence of on-site medical providers leads to frequent emergency department visits. With UHealth, Claiborne residents will benefit from daily access to medical professionals and 24/7 on-call coverage. This proactive approach allows for medical management within the facility, reducing unnecessary hospitalizations.

For short-term rehabilitation patients, UHealth clinicians offer frequent follow-up visits, medication reconciliation, and early discharge planning to ensure safe transitions home. They also provide clinical education to support staff and enhance the quality of care.
