The Covenant HomeCare rehabilitation team assists patients in becoming as functionally independent as possible. We coordinate our approach with multidisciplinary team meetings that incorporate patient and family concerns and goals into the professional plan of care. We have a proven track record of careful planning, goal setting, and effective, intensive one-on-one therapy. Our licensed, experienced staff evaluates and treats patients promptly after a physician's referral is received and we communicate regularly with the physician about a patient's progress. We also evaluate and assist in obtaining any prosthetic/orthotic, assistive device, or adaptive equipment needs.
